How to Get Started

When you are in pain, especially when it is debilitating, it can be overwhelming trying to find a place to get well and get signed up for treatment.  At Joliet Medical Center, we understand that. So we built this Getting Started Guide to help you.

Step 1:

Set up an appointment – There are 2 options for setting up your first appointment:

(1) Fill out the contact form on our contact page and we will contact you, or (2) call us at 815-714-2271 and our friendly office staff will help you.[/vc_column_text][/vc_column][vc_column top_margin=”none” width=”1/2″][vc_column_text css=”.vc_custom_1455573148146{padding-right: 50px !important;padding-left: 50px !important;}”]

Step 2:

Download and print out the intake forms, and then fill them out before your appointment (or come early and fill them out at the office).

(1) New Patient Form, (2) Consent to Treat, (3) HIPPA Notice of Privacy, (4) HIPPA Acknowledgement, (5) Minor Consent Form (for minors only)[/vc_column_text][/vc_column][/vc_row][vc_row top_margin=”none” el_id=””][vc_column top_margin=”none” width=”1/2″][vc_column_text css_animation=”” css=”.vc_custom_1453759547384{padding-left: 50px !important;}”]

Step 3:

Step 4:

Step 5:

Step 6:
